Please complete all the details of the enrolment form below, including medical information. As email is our preferred method of communication a valid email address is required. Class placement will only become valid once we receive your online Enrolment Form together with the payment of the Enrolment Fee. We are unable to hold a place in a class until this process is completed. We will acknowledge by email upon receipt of form and payment. An invoice will be emailed to you for Class Term Payments.
Solo students are to complete this form once you have confirmed your lesson time.
